Please use the various forms to the right to make requests to Facilities Management.
- To submit the Work Order Request Form, Key Request Form or Vehicle Request Form, simply click "Submit" when you are finished.
- To submit the Property Disposal/Relocation Request Form or the UPS Shipping Form, please complete the form and either email or fax it to Facilities Management at facilities@hendrix.edu or (501) 450-1211.
