Application Process

Hendrix faculty, staff (plus spouse, dependents) must complete a PAR-Q/ Health History Questionnaire found below and return to Sheila Rupert at ruperts@hendrix.edu. 

Community members must complete the following paperwork and return to Sheila Rupert, 1600 Washington Avenue, Conway, AR 72032 with ATTN: WAC/Athletic Department.

• Membership Application 
• Electronic Funds Transfer/Credit Card Authorization
• Guest Waiver & Release
