Assistance Animal Policy & Forms

The Assistance Animal Policy can be found below.  If you wish to apply for an assistance animal, please collect the documentation below. Necessary deadlines are listed in the Assistance Animal Policy.  If there are any issues uploading documents in the Assistance Animal Request Form, please email them to AssistanceAnimals@Hendrix.edu

Documentation for Application:

• Hendrix College Assistance Animal Policy - read this document in its entirety.
• Assistance Animal Request Form - Hendrix College account sign-in required.
• Medical Verification Form - verification must be with a medical provider outside of the Hendrix community.
• Service Animal Policy - applicable to service animals only.
  

All documentation and procedural requirements must be approved by the assistance animal committee before housing an assistance animal on campus.  Assistance animals are not pets but are considered a qualifying reasonable accommodation under the Americans with Disabilities Act in compliance with the Fair Housing Act.  Please refer to the Assistance Animal Policy for clarification on accommodation in compliance with federal laws and regulations.
