AGS Staples

REGISTRATION STEPS:

  1. Follow the link provided in your acceptance email and complete the online registration form by April 16, 2018.  
  2. Print, complete and sign the Parent-Student Signatures Assurance Form:  Parent-Student Registration Form  Both you and your parent/guardian MUST sign this form.
  3. If you have medical insurance, then make (1) one photocopy of your medical insurance card (front and back).  If you do not have medical insurance, then please email ags@hendrix.edu to notify us for your records.  Insurance is NOT required to attend AGS.
  4. Print, read and keep for your records the General Information document: General Information

  5. Send completed documents from registration steps #2 and #3 to AGS by April 16, 2018.  Documents may be sent to AGS in one of the following ways:

    • Scanned and uploaded to your online registration form (link provided in your acceptance email)
    • Scanned and emailed to ags@hendrix.edu (scanned documents should be combined into one (1) PDF)
    • Mailed in one envelope to AGS, 1600 Washington Ave, Conway, AR 72032
    • Faxed to 501-450-4579
